What to Expect

If You are Having a Dental Problem

Your first visit in our office will be to see the doctor. We will give you an examination, take necessary digital x-rays, and address your specific dental complaint. Treatment options and costs will be discussed. Depending on the problem, usually we can provide the necessary care that day. You should plan to pay for this visit at the time of service. If you have questions, please call us and we can estimate the costs for you.

If you have insurance, we will ask you to pay your deductible and co-pay at this visit. We can also estimate this with your insurance information. DO NOT FORGET YOUR INSURANCE INFO and CARD.

If You are Not Having a Dental Problem

Routine first visits in our office can be scheduled for an initial doctor exam or a doctor exam and initial dental cleaning with one of our dental hygienists. Appropriate digital x-rays will be taken, reviewed, and treatment will be recommended to meet your specific needs.

Previous Dental X-Rays

If you have dental x-rays at another dental office that are under three years old we request that you ask them to be forwarded to our office. You may download the X-RAY REQUEST FORM in the new patient forms area below. Print the form, fill in the information and forward it to the previous dental office.

When to Arrive

Please arrive 15 minutes before your scheduled appointment to complete necessary paperwork. You will be asked to complete the following forms:

  • Patient Registration Form
  • Medical History Form
  • List of Current Medications
  • Insurance Information (carrier and patient ID#’s are necessary)

You may download and print these documents using the links below. Please complete the forms, and bring them to your appointment or you may mail them to the office.
